Please review the following points and fill out the damage assessment form below.

• Any wall panel where the surface is not returned stripped of nails/screws or any foreign material (including vinyl signage) will incur an additional fee of $8 per running linear foot to return the wall to its original condition. No need to spackle holes; standard size nail/screw holes are perfectly acceptable.
• Any wall panel with damage due to: adhesive glue, direct wallpaper application or large holes in excess of .5” that can be repaired will incur a damage fee of $200 per each side of the affected panel to return the wall to its original condition.
• Replacement costs where no repair is possible is $500 per panel.
